    Offered here is an Atlas Combination Tail Stock and Carriage Lock / Tool Post Wrench. It is P/N 9-115 and I measure the sizes as 11/16" on the open end side and 3/8" on the square carriage lock side. This has been sitting around on a shelf for a while so a quick clean and polish wouldn't hurt. Aside from cosmetics the wrench looks to be in good condition.
